Each third of the face ages in a distinct fashion. Over the last two decades, treatment of the mid-third of the face, or midface, has received much attention in the surgical literature. As we age, the fullness of the cheek area (midface) starts to drop with gravity. This not only gives the cheek a hollow look, but it also changes the appearance of the lower eyelid and creates deep grooves around the mouth. While this area can be addressed by certain facelift techniques, some patients will benefit most from a directed midface lift.

A midface lift rejuvenates the cheek and lower eyelid area. It's performed through an endoscopic incision hidden in the hairline and diminishes the nasolabial folds, aging in the midface and may also improve the jowl area.

Technique

How is this surgery performed?

The procedure typically takes between 2-3 hours. Midface lifting is performed via an endoscopic approach using incisions hidden behind the hairline. As a midface lift is being performed, a lateral brow lift may also be performed simultaneously. As the soft tissue of the cheek is suspended to a more youthful position, the excess skin must be redistributed along the forehead and then into the hairline. Doing these procedures together allows a simultaneous rejuvenation of the cheek, the brow and the forehead. Often the lower eyelids (lower blepharoplasty) are addressed at the time of surgery to produce the most harmonious and youthful result.

What should I expect on the day of surgery and in the recovery period?

A midface lift with a brow lift and/or a blepharoplasty takes between two to three hours and can be performed under either twilight sleep (I.V. sedation) or general anesthesia. Prior to surgery you will meet the anesthesia team and be asked to review any allergies, as well as your major medical history. You'll be given a medication to help you relax. We work hard with the anesthesiologist to minimize any nausea or pain after surgery. After one to two hours of recovery you can be discharged from the surgery center with a family member or friend. We ask that someone stay with you for the first 24 hours after surgery. Although this procedure is minimally-invasive, initial recovery may take 2-3 weeks with improving results over several months.

Non-Surgical Facelift (Liquid Facelift & Ultherapy®)

There are options available for patients who wish to rejuvenate their appearance, but who may not wish to pursue surgery. One such option is what is often termed a liquid facelift which is a combination of muscle relaxing agents, such as Botox® Cosmetic or Dysport®, and dermal fillers, such as Juvederm®, Restylane®, Radiesse®, Sculptra® Belotero®, and Voluma®. These agents are used selectively depending on what you want to accomplish.

Botox® Cosmetic is used to treat wrinkles and folds that are due to muscle activity, such as between the brows or around the eyes and lips. When used correctly, patients can experience a natural brow lift that mimics a surgical brow lift while the medication lasts, typically between 3 to 6 months.

The use of dermal fillers has evolved dramatically over the past several years. While traditionally fillers were used to treat the deep folds and wrinkles that live in the texture of our skin, judicious placement of selected fillers can now be used to lift the volume of depleted areas of the face such as the cheeks, lips and jowls. It is so important that each treated area is not overdone. If just one feature looks unnatural, then the entire result suffers.

We now offer Ultherapy® the only FDA approved non-surgical, non-invasive treatment that uses ultrasound technology and the body's own natural healing process to lift and tighten loose skin on the eyebrow, neck, and under the chin. Ultherapy® uses ultrasound to stimulate the deep structural support layers of the skin without disturbing the surface of the skin.

There is no downtime with Ultherapy®, and patients can return to work right away. Results will unfold over the course of 2-3 months and some patients have reported continued improvement for up to 6 months, including lifting of the skin on the neck, under the chin, jawline and on the brow. While ultrasound does not duplicate the results of surgery, Ultherapy® has proven to be an inviting alternative for those who are not yet ready for surgery.

After your consultation, we work with you to decide on a treatment plan based on what areas are to be treated. A combination of oral medication, numbing cream and local anesthesia is often used to minimize any discomfort.
