Why L&P Aesthetics for SMAS Facelift Surgery?

At L&P Aesthetics, we don’t just perform facelifts—we specialize in the most advanced facial rejuvenation techniques available today. Our SMAS facelift approach represents the foundation of our renowned L&P Signature Deep Plane Facelift, the gold standard in facial plastic surgery that delivers natural-looking results lasting 10+ years.

Led by double board-certified facial plastic surgeons Dr. David M. Lieberman, Dr. Sachin S. Parikh, and Dr. Steven A. Curti, our team brings unparalleled expertise to every procedure. All three facial surgeons completed highly competitive fellowships specializing in facial plastic surgery, and they’re recognized nationwide as leaders in advanced facelift techniques. When you choose L&P Aesthetics, you’re choosing surgeons who understand facial anatomy at the deepest level—literally.

What sets our SMAS approach apart? Unlike outdated skin-only techniques that create a tight, “pulled” appearance and fade within just 2-5 years, our SMAS facelift addresses the root structural cause of facial aging. We lift and reposition the Superficial Musculoaponeurotic System—the muscular layer beneath your skin that truly supports your facial contours. This comprehensive approach produces dramatic yet natural improvements in your jawline, cheeks, and neck while maintaining your unique facial characteristics.

Our surgeons’ meticulous attention to detail extends to calculating the optimal vectors for lifting your face and neck. By working on the deeper SMAS layer rather than simply tightening skin, we achieve results that look refreshed and natural—never windswept or overdone. The tension is placed on strong muscular tissue, not your skin, which means your face moves naturally and ages beautifully from a more youthful starting point.

The L&P difference: Patients travel from across the country to experience our expertise in SMAS and deep plane facelifts. Our track record speaks for itself, and our surgeons are regularly recognized on New Beauty, Newsweek, and Castle Connolly lists of top surgeons. For the face, there’s truly nowhere else to go.

While we also offer traditional SMAS techniques for patients at earlier stages of aging, the L&P Signature Deep Plane Facelift is the true gold standard—and what delivers the exceptional, longest lasting transformation most patients seek when they’re ready to invest in the best facial rejuvenation.

L&P Signature Deep Plane Facelift and Neck Lift

This patient had out L&P Signature Deep Plane Facelift and Neck Lift with tissue contouring, a lower blepharoplasty, a limited fat transfer, and a chin implant. Revision Lift Complimented with a Fat Transfer

You would never believe that this patient was in her late 60s! This beautiful patient is approximately five months out from her revision facelift and neck lift. This was a unique case because she had a previous face and neck lift with an outside surgeon a few years ago. She wanted to improve the laxity along her neckline and jawline. We also thought she could benefit from subtle volumetric improvements in her face so a fat transfer was the perfect compliment. The combination of procedures helped restore the natural volume in her face enhancing a more youthful appearance. You can see the improved transition from her lower eyelid to cheek junction, natural fullness in her cheekbones, and a sharp contour along her jawline. This is a wonderful example demonstrating that when this surgery is done well, people can look effortlessly beautiful.

What Is a SMAS Facelift?

A SMAS facelift is a validated approach to facial rejuvenation, targeting the Superficial Musculoaponeurotic System—a complex network of muscle and connective tissue that lies beneath your skin. Think of the SMAS as the foundational scaffolding of your face. When this layer weakens and descends with age, it creates the visible signs we associate with aging: jowls, deep smile lines, sagging cheeks, and a less defined jawline.

Here’s what makes SMAS facelift surgery revolutionary: Instead of simply tightening loose skin (which provides only temporary improvement), a SMAS facelift lifts and repositions this deeper muscular layer, correcting the actual anatomical cause of facial aging. However, there’s an even more advanced approach. The SMAS facelift, revolutionary when introduced in the 1970s, laid the groundwork for what is now considered the true gold standard—the Deep Plane Facelift. At L&P Aesthetics, our proprietary L&P Signature Deep Plane technique takes the SMAS concept to its ultimate evolution, working beneath the SMAS layer for even more comprehensive, natural, and longer-lasting results (12-15+ years). The Deep Plane Facelift technique is mastered by only a select group of surgeons nationwide, including Dr. Lieberman, Dr. Parikh, and Dr. Curti.

Your SMAS Facelift Journey at L&P Aesthetics

In staying up to date with the latest advances in facial cosmetic surgery, facelift specialists Dr. David M. Lieberman, Dr. Sachin S. Parikh, and Dr. Steven A. Curti take each patient’s facial features into account when crafting a surgical plan. This extends to calculating the optimal vectors for lifting the face and neck to achieve as natural a result as possible; tightening the facial skin using the correct angle can deliver the ideal surgical result for each individual patient. This intricate nuance will help the results appear as natural as possible while minimizing such undesired effects as pleating of the skin near the ears, bunching of the skin under the chin or the mid-face, or flattening of the mid-face that can occur if the skin is pulled in an overly vertical or horizontal direction.

The Consultation Process

Your journey begins with a comprehensive consultation where we analyze your facial structure, discuss your aesthetic goals, and create a customized surgical plan. Our surgeons take time to understand not just what bothers you about aging, but how you envision your refreshed appearance.

The Surgical Experience

Your SMAS facelift is performed at our state-of-the-art Alma at Addison Surgery Center under the care of certified anesthesiologists. The procedure typically takes 3-4 hours, during which our surgeons:

  1. Create carefully concealed incisions along natural creases
  2. Lift and reposition the SMAS layer using your predetermined optimal vectors
  3. Remove excess skin without tension
  4. Close incisions with meticulous technique for minimal scarring

Most patients return home the same day with comprehensive aftercare support from our team.

Your transformation continues to refine as tissues settle into their new position, revealing your complete rejuvenation.

Results & Longevity

Expect a crisper jawline, lifted cheeks, and a smoother neck—a rested, confident and the best version of you. Because we reinforce the deep foundation, SMAS outcomes are durable (commonly 10–12 years, varying by skin quality, genetics, and lifestyle). You’ll continue to age gracefully—from a more youthful baseline.

Maintain your results with diligent SPF, healthy habits, and periodic nonsurgical care (BBL®, Morpheus8, microneedling, neuromodulators, and medical skincare curated by our team).

L&P Signature Deep Plan difference: While both techniques deliver natural results, the L&P Signature Deep Plane approach provides more comprehensive correction (especially mid-face), longer-lasting results (5+ additional years), and the most natural appearance possible because of zero skin tension.

SMAS vs. Deep Plane vs. Mini: Which Facelift Fits You?

There is no one “best” facelift — only the best match for you. During consultation, we’ll map your anatomy, skin quality, and priorities to a technique.

  • SMAS Facelift
Works on top of the SMAS layer to tighten and reposition it. Highly customizable (plication, SMASectomy, extended/high‑SMAS). Excellent for lower‑face and neck definition with a reasonable recovery.
  • Deep Plane Facelift (L&P Signature Facelift) – Gold Standard
Works beneath the SMAS, lifting the skin and SMAS together after releasing deeper ligaments. Offers the most comprehensive elevation (especially the midface) with natural expression and durable support—often our recommendation when you want the widest scope of rejuvenation. Our hallmark technique for comprehensive rejuvenation. Results last 12-15+ years (5+ years longer).
  • Mini/Short‑Scar Facelift
Limited dissection for early jowling and mild laxity. Faster recovery, more subtle change, and shorter longevity. Often combined with nonsurgical treatments.

We will explain your options clearly and recommend the approach that balances effectiveness, safety, longevity, downtime, and your goals.

Are You an Ideal Candidate for a SMAS Facelift?

You may be a good SMAS facelift candidate if you:

  • See moderate to significant lower‑face and neck aging (jowls, softened jawline, neck laxity)
  • Want a natural refresh without changing who you are
  • Are in good overall health and a non‑smoker (or willing to stop 4–6 weeks before and after)
  • Have some skin elasticity to redrape smoothly over the new foundation

During your consultation, we’ll assess vector directions, skin quality, fat distribution, and ligament support to personalize your plan.

Facelift Recovery and Beyond

While surgical facelifts require more recovery time, their results are notably longer-lasting. However, it’s important to understand that while a facelift can significantly rejuvenate your appearance, it does not halt the natural aging process. Patients may still notice new signs of aging as they continue to age.

Frequently Asked Questions About SMAS Facelifts

The term “facelift” is broad and can refer to various techniques. Outdated “regular” facelifts often manipulated only skin, providing temporary, unnatural results. A SMAS facelift specifically targets the deeper Superficial Musculoaponeurotic System—the muscular layer beneath your skin—addressing the root structural cause of facial aging. This approach delivers more natural, longer-lasting results (10-12 years vs. 2-5 years for skin-only techniques).

When performed by experienced facial plastic surgeons like Dr. Lieberman, Dr. Parikh, and Dr. Curti, SMAS facelift produces exceptionally natural results. The key is placing tension on the deep SMAS layer rather than on your skin. This allows your skin to drape naturally without tightness, preserving your facial expressions and unique characteristics. Most people will simply think you look well-rested, refreshed, or “like you’ve been taking great care of yourself”—not that you’ve had surgery.

Scientific studies consistently show that well-performed SMAS facelifts last an average of 10-12 years, with some patients enjoying results for 15 years or more. This is significantly longer than skin-only facelifts (2-5 years) or mini facelifts (5-7 years). Factors affecting longevity include your age at surgery, skin quality, genetics, sun protection practices, and lifestyle factors like smoking and weight stability.

Both techniques address deeper facial structures, but the deep plane facelift (like our L&P Signature Facelift technique) goes even further, working beneath the SMAS layer and releasing retaining ligaments. SMAS facelift works on top of the SMAS, separating it from the skin before tightening. Deep plane typically provides longer-lasting results (12-15+ years) and exceptional mid-face rejuvenation, but requires more extensive surgery. Your surgeon will recommend the optimal approach based on your specific anatomy and goals.

A mini facelift is a less extensive procedure suited for younger patients with mild aging signs—primarily addressing the jawline with limited skin removal and SMAS manipulation. It offers shorter recovery but less dramatic, shorter-lasting results (typically 2-5 years). A full SMAS facelift provides comprehensive correction of moderate to significant aging in the face and neck with results lasting 10-12 years. We’ll recommend the approach that best matches your needs.

A liquid facelift uses injectable treatments like BOTOX Cosmetic and dermal fillers to temporarily smooth wrinkles and restore volume. Results last months (not years) and require ongoing maintenance. While excellent for early aging signs, nonsurgical options cannot address significant sagging or reposition descended tissues. A SMAS facelift provides structural correction of jowls, sagging cheeks, and neck laxity with results lasting over a decade—making it the gold standard for moderate to advanced facial aging.

Most patients are pleasantly surprised that pain is quite manageable. Rather than sharp pain, you’ll experience tightness, numbness, and mild discomfort in the first few days. Prescribed pain medication keeps you comfortable initially, and most patients transition to over-the-counter options by days 4-6. The sensation of tightness gradually improves over the first few weeks. The first 3-4 days are typically the most uncomfortable, then improvement is rapid.

Timeline varies by individual and job requirements:

  • Desk work: Most patients return after 2-3 weeks
  • Physical work: 4-6 weeks, depending on demands
  • Social activities: Many feel comfortable by week 3 with makeup
  • Light exercise: Walking can begin immediately; gentle cardio at 3-4 weeks
  • Strenuous exercise: Wait until week 6 or as cleared by your surgeon
  • Complete healing: 3-6 months for final results and full scar maturation

Dr. Lieberman, Dr. Parikh, and Dr. Curti place incisions strategically in natural creases and hairlines to maximize camouflage. Incisions run along the temples (in the hairline), in front of the ears (following natural creases), around the earlobes, and behind the ears into the lower scalp. With proper healing and scar care, most SMAS facelift scars fade to thin, nearly imperceptible lines that are very difficult to detect once mature (12-18 months). Patients routinely wear their hair up or styled away from their face without any concerns about visible scarring.

Absolutely! Many patients combine SMAS facelift with complementary procedures for comprehensive facial rejuvenation:

During your consultation, your surgeon will discuss which combination best achieves your aesthetic goals.

There’s no specific age requirement. The ideal time for a SMAS facelift is when you have moderate to significant signs of aging in your lower face and neck—typically in your 40s through 70s. However, we’ve successfully performed facelifts on patients in their 30s (with early aging) and their 80s (with good health). What matters most is your health status, tissue quality, and realistic expectations.

The most important factors when looking for a SMAS facelift surgeon are:

  • Board certification in facial plastic surgery or plastic surgery
  • Specialized fellowship training in facial plastic surgery
  • Extensive experience performing SMAS and deep plane facelifts specifically
  • Before and after photos demonstrating natural-looking results
  • Patient reviews emphasizing satisfaction and care quality
  • Hospital privileges and an accredited surgical facility
  • Strong communication and alignment with your aesthetic goals

Dr. Lieberman, Dr. Parikh, and Dr. Curti meet and exceed all these criteria, which is why patients trust them with their faces from across the country.

Our double-board certified facial plastic surgeons—Drs. Lieberman, Parikh, and Curti—bring Stanford-trained expertise and nationally recognized skill to every procedure. They’ve mastered multiple SMAS techniques and customize the approach to your unique anatomy.
